This form is designed to assist the FBC Ministry to better prepare for all who minister the gospel for whatever program or service it may require.

This form is for the sole use of the FBC Ministry. All ministries are required to comply.
FURNERAL/WAKES CONTRACTS
All payments must be withcash, a cashiers check or money order - personal checks will not be accepted.
FACILITY CONTRACT
This contract is made available for use of the Fellowship Hall for limited occassions.
The date and time must be provided so that we will not book anyone that conflict with FBC calendar of events.

Activity Form
This form is designed for any ministry or committee to fill out for programs or special activities.
All activities must be approved before planning is done, to ensure that there is not a conflict with the calendar of events, and in accordance with FBC Ministries.
YPLEDGE REGISTRATION FORM
This form is to collect pledges or donations from friends, family, and co-workers. You can collect checks or cash. Credit card pledges can be done by entering the information on our website tithes/donation page, or you can pay by check to the FIRST BAPTIST CHURCH and please enter “#walkathon” in the memo.
The registration fee is only $5.00 per person. This can be prepaid or on sight the day of this great event and cause.All monies collected will sent to the “Breast Cancer Research Foundation”.
